In order to guarantee the safety, effectiveness, and quality of the care we offer, it is essential that all patients fill out the required medical paperwork prior to or at the time of their appointment. This paperwork will allow our staff to determine your care needs in order to offer adequate care regarding consultations on IUDs (intrauterine devices) and other contraception alternatives.

For new patients, the New Patient Form is to be completed, which comprises important details, some of which include gynaecological and contraception information concerning the insertion, removal, and follow-up visit of the IUD. Similarly, a patient undergoing a flu shot is to complete the Flu Vaccine Consent Form prior to treatment. For patients attending the clinic concerning occupational injuries, the WorkCover Questionnaire is to be completed.

For the purpose of continuity of care, the Medical Record Transfer Form enables us to request securely your past medical information, including previous treatment with the IUD. We have a Fee List that clearly indicates the cost of consultation and the out-of-pocket expense for services such as IUD consultation or surgery.

By filling out these forms in advance, waiting time can be shortened and communication between you and your healthcare provider can improve.
